🏥 Insurance & Payment Information

• Important Update (2026): Due to the continuation of declining reimbursement rates and rising costs of reconstructive care, we are no longer in-network with any plans.

• Our Practice Status: For private insurance plans (Anthem Blue Cross, Blue Shield, Aetna, Cigna, United): We are exclusively an out-of-network (OON) provider.

• Payment for all services — including office visits and surgical procedures — is required in full at the time the service is rendered.

• We will not bill your private insurance company directly for any service.

📝 Patient Reimbursement Process (The Superbill)

• To ensure you can maximize your out-of-network benefits, we will provide you with a detailed Superbill and all necessary documentation for your services.

• The patient is responsible for submitting this Superbill to the insurance company for direct reimbursement or for applying the fee toward their out-of-network deductible.
