🏥 Insurance & Payment Information
Important Update (2026): Due to the continuation of declining reimbursement rates and rising costs of reconstructive care, we are no longer in-network with any plans.
Our Practice Status: For private insurance plans (Anthem Blue Cross, Blue Shield, Aetna, Cigna, United): We are exclusively an out-of-network (OON) provider.
Payment for all services — office visits ($50) and surgical procedures (varies) — is required in full at the time the service is rendered.
We will not bill your private insurance company directly for any service.
📝 Patient Reimbursement Process (The Superbill)
To ensure you can maximize your out-of-network (OON) benefits, we provide you with a detailed Superbill and all necessary documentation for your services.
The patient is responsible for submitting this Superbill to the insurance company for direct reimbursement or for applying the fee toward their out-of-network deductible.

Allē is Allergan’s loyalty rewards program for patients (the company that makes Botox). Signing up is free, and members earn points on eligible services and redeem them for savings. The system rewards loyalty through points accumulation, exclusive offers, and tiered VIP benefits.
Allē is partnered with Cherry, and members can finance eligible treatments and products through flexible monthly installments directly via the Allē app. Patients apply with a quick soft credit check — no hard inquiry — and can receive approval for up to $10,000 selecting plans starting at 0% APR for those qualified.
