Erbium Laser Resurfacing
Maximize Results, Minimal Downtime: the Next Generation of Precise Ablative Treatment
Not all lasers are created equal.
Erbium laser resurfacing — using the Sciton Joule Er:YAG platform — represents the most technically refined approach to skin resurfacing available today: deeper results than non-ablative treatments, with a recovery profile that makes the older generation of CO2 lasers look blunt by comparison.
At the Dērm Boutique, every laser treatment is performed personally by Dr. Brock Lanier — a double-board-certified surgeon with the anatomical expertise and clinical judgment to calibrate each treatment precisely to your skin, your goals, and your life.
~ Why Erbium is Different? ~
Precision Where It Counts
Laser resurfacing works by removing damaged skin layers and triggering a profound wound-healing response — stimulating new collagen production, tightening the dermis, and revealing healthier skin beneath. The critical variable is not whether a laser can do this, but how much collateral heat damage it leaves behind in the process.
This is where the Erbium laser has a decisive advantage over older carbon dioxide/CO2 technology. The Erbium wavelength contacts the skin more efficiently — 10x more efficiently compared to the CO2 wavelength. The result is a cleaner, more controlled treatment: the laser energy is consumed almost entirely in the targeted tissue, rather than conducting heat into the neighboring skin. In practical terms, the Erbium laser causes less heat damage and this translates into shorter recovery, less redness, lower risk of pigmentation complications. Less pain. Faster recovery.
This is not a newer or less powerful technology. It is a more precise one.
Erbium vs CO2 lasers
An Honest Comparison:
Understanding Your Options
CO2 lasers have a long history in aesthetic medicine and remain a valid choice for certain patients. For those with very light skin tones and with deeply etched wrinkles and significant skin laxity, the CO2 laser’s deep heat injury can produce a strong collagen contraction.
But for the majority of patients seeking meaningful skin rejuvenation with a realistic recovery timeline, CO2's legacy is also its limitation. It higher heat injury is responsible for weeks of redness as well as a significant risk of permanent pigmentation changes. Even with modern fractional lasers (like the Fraxel), CO2 carries a higher pain profile, longer redness, and stricter patient-selection requirements.
The Erbium laser was developed specifically to address these problems. Clinical studies comparing the two technologies confirm what patients experience: Erbium lasers produce results with significantly lower pain scores, redness subsiding faster, and a broader suitability across skin tones.
At the Dērm Boutique, we use the Sciton Erbium platform precisely because it allows Dr. Lanier to deliver ablative-level results with the control and safety margin that a surgeon-led boutique practice demands.
What Erbium laser resurfacing can treat…
-
By removing damaged epidermal and dermal tissue and stimulating new collagen synthesis, Erbium resurfacing visibly softens fine to moderate lines — particularly perioral lines, crow's feet, and lines across the forehead and neck that topical treatments cannot meaningfully address.
-
Fractional Er:YAG disrupts the fibrotic architecture of scar tissue and stimulates healthy collagen deposition, progressively reducing scar depth and improving surface texture. Clinical studies show significant improvement in standardized scar grading scores after a series of treatments.
-
Years of UV exposure manifest as textural irregularity, dyspigmentation, enlarged pores, and generalized dullness. Erbium resurfacing addresses the structural causes of these changes, not just the surface appearance. Emerging research also indicates that ablative resurfacing clears actinic damage and precancerous cells, with studies showing a meaningful reduction in subsequent keratinocyte carcinomas in treated patients.
-
Uneven texture, rough skin surface, and generalized photoaging all respond well to Erbium treatment, with the normalization of the epidermal-dermal architecture producing a smoother, more luminous result.
-
Often the first areas to betray aging, and frequently undertreated. Erbium resurfacing is well-suited to these thinner-skin areas where the controlled thermal profile reduces the risk of complications.

Why We Use Sciton
The Sciton Platform
Not all lasers are equivalent. The delivery system, the precision of energy calibration, and the quality of the fractional scanner all determine how consistently and safely a result can be achieved.
The Sciton Erbium platform is among the most clinically respected systems available. It offers fully adjustable pulse durations, tunable ablation-to-coagulation ratios, and fractional delivery options that allow Dr. Lanier to customize each treatment with precision.
Where older or less sophisticated Erbium systems were limited by an inability to coagulate tissue at depth, the Sciton's variable technology allows for controlled dermal remodeling that matches — and in many cases exceeds — the clinical outcomes of CO2 with a meaningfully better safety and recovery profile.
Preparing & Protecting: Pre- & PostCare
Before your treatment:
For two to four weeks prior, a skincare regimen including retinoids and antioxidants primes the skin and enhances the healing response. These actives should be discontinued 5-7 days before your treatment.
Avoid blood-thinning supplements (fish oil, Vitamin E) for one week prior, and arrive with clean skin free of makeup and sunscreen.
After your treatment:
In the first 24–48 hours, keep the skin clean and calm — a gentle cleanser and barrier-supporting moisturizer only. No active ingredients.
As re-epithelialization progresses, growth factor serums can be introduced to accelerate recovery and amplify collagen production. Retinoids and exfoliating acids should be avoided for 2-4 weeks.
Sun protection is non-negotiable for a minimum of 4 weeks post-treatment. Newly formed skin is highly susceptible to UV damage and pigmentation changes, and the results you've invested in deserve to be protected.
Is It Right for You?
Candidacy & Contraindications
Erbium laser resurfacing is appropriate for most adults in good general health seeking meaningful improvement in skin texture, tone, scarring, or signs of aging. Its thermal profile makes it significantly safer than CO2 for patients with Fitzpatrick skin types III–V (darker skin types).
Ideal candidates are those with fine to moderate wrinkles, acne scarring, sun damage, or uneven texture who want results that go beyond what non-ablative treatments can deliver — without the extended recovery associated with CO2.
Treatment should be deferred if you have an active skin infection, cold sores in the treatment area, or inflammatory acne.
Patients with a history of herpes simplex can be safely managed with antiviral prophylaxis.
Those on isotretinoin (Accutane) should discuss timing with Dr. Lanier directly — current evidence suggests the traditional six-month waiting period may be more conservative than necessary, though a personalized assessment is always appropriate.
For patients with very deep rhytids or severe skin laxity who are seeking maximal single-session results and can accept longer recovery, Dr. Lanier will discuss whether a different protocol or modality may better serve their goals.
Common Questions: FAQ
-
The Erbium wavelength is absorbed by water in the skin more than ten times more efficiently than CO2. This means the laser energy is consumed almost entirely in the targeted tissue rather than spreading heat into the surrounding dermis. The result is a controlled ablation with significantly less thermal damage — translating into faster healing, less prolonged redness, and a much safer profile for patients with darker skin tones.
-
For lighter fractional treatments, most patients return to normal activity in three to five days, with redness resolving within one to two weeks. Deeper treatments designed to address significant scarring or photoaging may involve seven to ten days of visible recovery. Dr. Lanier will give you a precise, realistic expectation based on the protocol appropriate for your skin.
-
It depends on the concern and the intensity of treatment. A single deeper session may address significant photoaging or scarring. A series of lighter passes spaced four to six weeks apart is an excellent approach for ongoing skin refinement with minimal social disruption — and is the foundation of our membership-based treatment model.
-
Erbium resurfacing has a significantly better safety profile for patients with Fitzpatrick types III–V compared to CO2. The limited thermal diffusion dramatically reduces the inflammatory cascade that triggers melanin overproduction. Dr. Lanier performs a thorough skin assessment at every consultation to ensure the appropriate protocol for your specific skin type.
-
Laser resurfacing outcomes are directly tied to clinical judgment — the selection of depth, pulse duration, number of passes, and energy density are decisions that require both technical expertise and anatomical understanding. A double-board-certified surgeon who operates daily brings a level of precision and accountability that a technician following a protocol simply cannot replicate. The difference is not a marketing claim. It is a clinical one.
